Replacing a Tag

If your pet’s QR tag has been damaged or lost, please complete the “Replacement Tag Form” from this link.

There is a $10.00 service charge for all replacement tags, payment is processed in the form. Once the form is submitted your new tag will be sent out promptly. The new tag will arrive ready for use and will already have all of your pet’s information on it. There will be no additional setup steps required.
